can you please provide an explanation as to why my request for an Autism
Chaperone, made under the Autism Act 2009, was completely ignored for the
26th October A&E attendance?

due to brain damage caused by being a victim of 16 years Domestic Violence
i was unable to recall as part of the complaint that i had photographed the
handwritten note.

please ensure that you cc the Parliamentary Ombudsman citing Ref C-2152382.
