at 30,000,000 instructions there is a dcache bug triggered.  a request
comes in: two cycles later d_out.valid is set and some (random-looking,
but actually an instruction) data is outputted.  however the *actual*
valid data is outputted *another two cycles later*

this will take a few days to track down.
