More updates:

sv_binutils
* dropped reliance on opcodes; from now on, we take the first insn name we met,
and don't bother checking it one more time;
* introduced a per-record validity flag; for now, we only check whenever we
attempt to bind an SVP64 recprd which was already bound;

binutils-gdb
* updated the generated files respectively;
* introduced svp64_setup_opcodes routine which takes care of binding
svp64_record.
