Pushed few more commits, now it looks like the whole code generation is
format-based. I don't quite like the part where we convert CSV entry into (op,
insn, row) stuff, this should likely be in some function or whatever, but maybe
later. Anyway, I think the code is ready to output what we need for binutils.
