instructions:

* prefixed load/store immediate byte thru quad
* paddi
* cntlzdm and trailing (masked)
* cfuge, pextd, pdepd
* brh brw brd - these are actually all covered by grevlut
* setbc, setbcr, setnbc etc.
* pnop - prefixed nop
* prefixed FP load/store immediate

approximately 45 instructions, only 14 of which are new, the others
extend existing instructions.
