thanks, I'll be very glad if the outcome of this question is that N without M
is just a mistake in the tables.  There are occurrences of them in the 16-bit
only tables under 2.1.6, 2.1.7 and 2.1.8.

If it wasn't a mistake, 2.1.1 would probably have to have extra lines in the
encoding formats table covering these cases of M-less Ns, and that would make
for additional complexity in both phase 1 and phase 2 of the decoder.
